• Introducción a la narcolepsia en la infancia

    NARCOLEPSIA
    La narcolepsia es un trastorno crónico con graves consecuencias en la calidad de vida y que debuta típicamente en la adolescencia, aunque un tercio de los pacientes describen el inicio de los síntomas en la infancia (Nevsmalova S, 2009). Es una enfermedad rara, que afecta a un 0.25-0.56% de la población, sin predominio de sexo y con dos picos de incidencia (15 y 30-35 años) (Silber et al, 2002). Aunque la narcolepsia familiar es rara y la mayoría de los casos son esporádicos, los familiares de primer grado de pacientes narcolépticos tienen 40 veces más riesgo de desarrollar narcolepsia (Nishino et al, 2000).
    Se caracteriza por 5 síntomas: SDE, cataplejia, alucinaciones hipnagógicas e hipnopómpicas, parálisis de sueño y un sueño nocturno fragmentado (ICSD, 2005). La enfermedad tiene un origen autoinmune (Hallmayer et al, 2009) y el HLA DQB1*0602, presente en un 70% de estos niños, indica susceptibilidad a la enfermedad, que aparece si existen factores externos (infecciones, traumatismos, etc) (Dauvilliers et al, 2011; Thannickal et al, 2000). En la mayoría de los pacientes con cataplejia existe un déficit de hipocretina-1 en líquido cefalorraquídeo (LCR) (Nshino et al, 2000), por afectación hipotalámica.
    Algunos trastornos médicos que afectan al SNC pueden provocar una narcolepsia secundaria o “sintomática” de inicio precoz: Síndrome de Prader-Willi, Distrofia Miotónica tipo 1, Síndrome de Down, traumatismos, tumores intracraneales, trastornos vasculares o infecciones.
