3 METILGLUTACONIL ACIDURIA TIPO I
- Etiología
Déficit 3 metilglutaconil CoA hidratasa, enzima que participa en el metabolismo de la leucina. Herencia autosómica recesiva.
Prevalencia Menos de 20 casos descritos
- Clínica
Debut en periodo nenatal o infancia precoz en forma de retraso psicomotor o del lenguaje. Pueden presentar también hipoglucemia, microcefalia, trastorno de la alimentación, vómitos, epilepsia, atrofia de los ganglios basales, PCI, distonía, hipotonía y hepatomegalia.
- Diagnóstico
Análisis de ácidos orgánicos: niveles muy elevados de acido 3 metilglutacónico y ácido 3 hidroxiisovalérico.
Estudio de la actividad de la 3 metil glutaconil CoA hidratasa en cultivo de fibroblastos o leucocitos.
- Tratamiento
Dietético: restricción de leucina y suplementación con L carnitina
